Changing Your Name Introduction Documents you need to change your name

The documents you'll need for the registration process at ICA are as follows:-

Compulsory items:

Existing IC; and
1 recent colour photo taken against white background with matt or semi-matt finish (35mm x 45mm)

and any 1 of these documents (depending on reason):

Deed Poll for change or deletion of name; or
Baptism Certificate for insertion of baptism name; or
Religious Certificate for insertion of religious name; or
Marriage certificate for insertion of married name only.
